Consultation

Examination
A thorough physical exam including range of motion, orthopedic testing, and neurological evaluation to pinpoint the issue.
Treatment Plan
Based on findings, Dr. Klein will explain what’s going on and outline a personalized plan to get you out of pain.
Before Your Visit
What to Bring
To make your first visit as smooth as possible, please bring the following items to your appointment.
- Photo ID (driver’s license or state ID)
- Insurance card (front and back)
- Completed intake forms (download below)
- List of current medications
- Any relevant imaging (X-rays, MRI reports)
- Referral paperwork (if applicable)

Will I receive treatment on my first visit?
